Integral to the diagnosis of autism spectrum disorder is the initial assessment through which the existence of a ‘problem’ is first ascertained. Despite this, there remains limited research on this early part of the diagnostic pathway. In this article, we utilised conversation analysis to examine relevant issues in relation to the practitioner–family interactions that take place within this initial assessment context. Our findings illustrated that parents typically first raised the possibility of the presence of an autism spectrum disorder diagnosis through (...) ‘building a case’, which professionals were then able to ratify or negate. Furthermore, we found that the assessments unfolded sequentially and clinical decisions were typically reached through a distinctive pattern of interaction. These findings have important implications for clinical practice, including for the study of autism spectrum disorder assessments and diagnosis. (shrink)

Compared to our understanding of positive prediction error signals occurring due to unexpected reward outcomes, less is known about the neural circuitry in humans that drives negative prediction errors during omission of expected rewards. While classical learning theories such as Rescorla–Wagner or temporal difference learning suggest that both types of prediction errors result from a simple subtraction, there has been recent evidence suggesting that different brain regions provide input to dopamine neurons which contributes to specific components of this prediction error (...) computation. Here, we focus on the brain regions responding to negative prediction error signals, which has been well-established in animal studies to involve a distinct pathway through the lateral habenula. We examine the activity of this pathway in humans, using a conditioned inhibition paradigm with high-resolution functional MRI. First, participants learned to associate a sensory stimulus with reward delivery. Then, reward delivery was omitted whenever this stimulus was presented simultaneously with a different sensory stimulus, the conditioned inhibitor. Both reward presentation and the reward-predictive cue activated midbrain dopamine regions, insula and orbitofrontal cortex. While we found significant activity at an uncorrected threshold for the CI in the habenula, consistent with our predictions, it did not survive correction for multiple comparisons and awaits further replication. Additionally, the pallidum and putamen regions of the basal ganglia showed modulations of activity for the inhibitor that did not survive the corrected threshold. (shrink)

Nursing Inquiry 2012 [Epub ahead of print] Include, differentiate and manage: gay male youth, stigma and healthcare utilizationIn Canada, there has been a recent increase in HIV incidence among young men who have sex with men. However, gay male youth (GMY) may forego HIV testing due to fear of stigmatization. Therefore, the aim of this research was to explore the perceptions of stigma in health care within this population. The research was conducted through a series (...) of semi‐structured interviews with eight GMY aged 20–29, who frequented a gay‐friendly clinic in downtown Ottawa, Canada. In a sub‐analysis of the interviews utilizing the work of Hardt and Negri’s three‐part sequence – inclusion, differentiation and management – we found that homosexuality‐related stigmatization affects the interaction between GMY and health professionals. Interview participants perceived HIV‐related risk counselling as a manifestation of stigma. These findings reveal that the experience of stigma and stigmatization is not necessarily based on health professionals’ intentions, but rather, on patients’ perceptions of the interactions they have with health professionals. Specific modifications to the delivery of health care may help to overcome stigmatization. (shrink)

Background Policy decisions about childhood vaccination require consideration of multiple, sometimes conflicting, public health and ethical imperatives. Examples of these decisions are whether vaccination should be mandatory and, if so, whether to allow for non-medical exemptions. In this article we argue that these policy decisions go beyond typical public health mandates and therefore require democratic input.Methods We report on the design, implementation, and results of a deliberative public forum convened over four days in Ontario, Canada, on the topic of childhood (...) vaccination.Results 25 participants completed all four days of deliberation and collectively developed 20 policy recommendations on issues relating to mandatory vaccinations and exemptions, communication about vaccines and vaccination, and AEFI (adverse events following immunization) compensation and reporting. Notable recommendations include unanimous support for mandatory childhood vaccination in Ontario, the need for broad educational communication about vaccination, and the development of a no-fault compensation scheme for AEFIs. There was persistent disagreement among deliberants about the form of exemptions from vaccination (conscience, religious beliefs) that should be permissible, as well as appropriate consequences if parents do not vaccinate their children.Conclusions We conclude that conducting deliberative democratic processes on topics that are polarizing and controversial is viable and should be further developed and implemented to support democratically legitimate and trustworthy policy about childhood vaccination. Academic incivility can create divisiveness and affect learners’ sustainability towards degree attainment. Therefore, there is a need to empirically assess learner views on what constitutes academic incivility in higher education. To fill this gap, we developed the Incivility Indicators Instrument (i3), a multifaceted scale that measures the extent to which students view cumulative behaviors and dynamics as instances of academic incivility. To examine the reliability and factor structure of the i3, we conducted a two-part study with two samples of adults (...) in the United States who were enrolled in higher education programs. In study 1 (N = 342), we conducted an exploratory factor analysis (EFA) and found the initial three-factor structure. In study 2, we administered the instrument to a validating sample (N = 510) and examined the stability of the three-factor model and scores through confirmatory factor analysis (CFA). In this study, we describe the results of the study, provide implications, and discuss future directions. (shrink)

Clinical trials emerged in rapid succession as the COVID-19 pandemic created an unprecedented need for life-saving therapies. Fair and equitable subject selection in clinical trials offering investigational therapies ought to be an urgent moral concern. Subject selection determines the distribution of risks and benefits, and impacts the applicability of the study results for the larger population. While Research Ethics Committees monitor fair subject selection within each trial, no standard oversight exists for subject selection across multiple trials for the same disease. (...) Drawing on the experience of multiple clinical trials at a single academic medical centre in the USA, we posit that concurrent COVID-19 trials are liable to unfair and inequitable subject selection on account of scientific uncertainty, lack of transparency, scarcity and, lastly, structural barriers to equity compounded by implicit bias. To address the critical gap in the current literature and international regulation, we propose new ethical guidelines for research design and conduct that bolsters fair and equitable subject selection. Although the proposed guidelines are tailored to the research design and protocol of concurrent trials in the COVID-19 pandemic, they may have broader relevance to single COVID-19 trials. (shrink)

Neste trabalho faz-se uma relação entre as informações encontradas na literatura e a atualidade na Terra Indígena Kaingang em relação ao conhecimento que possuem sobre o uso de plantas para a cura de doenças e como interpretam a ação da erva no organismo. Para isto, realizou-se uma pesquisa de campo em uma Terra Indígena Kaingang no Paraná, cujo entrevistas forneceram (...) confirmações históricas em relação aos etnoconhecimentos da etnia, da evolução dos aspectos químicos e das atitudes perante a química e os remédios de origem farmacêutica e natural. Pancreatitis is a complex, progressively destructive inflammatory disorder. Alcohol was long thought to be the primary causative agent, but genetic contributions have been of interest since the discovery that rare PRSS1, CFTR and SPINK1 variants were associated with pancreatitis risk. We now report two associations at genome-wide significance identified and replicated at PRSS1-PRSS2 and X-linked CLDN2 through a two-stage genome-wide study. The PRSS1 variant likely affects disease susceptibility by altering expression of the primary trypsinogen gene. The CLDN2 risk allele is (...) associated with atypical localization of claudin-2 in pancreatic acinar cells. The homozygous CLDN2 genotype confers the greatest risk, and its alleles interact with alcohol consumption to amplify risk. Background Noninvasive prenatal testing (NIPT) provides important benefits yet raises ethical concerns. We surveyed Canadian pregnant women and their partners to explore their views regarding pressure to test and terminate a pregnancy, as well as other societal impacts that may result from the routinization of NIPT.Methods A questionnaire was offered (March 2015 to July 2016) to pregnant women and their partners at five healthcare facilities in four Canadian provinces.Results 882 pregnant women and 395 partners completed the survey. 64% of women (...) anticipated feeling no pressure to take the test if it were offered routinely, and 39% were not concerned about routinization leading to increased pressure to terminate a pregnancy of a fetus with Down Syndrome. Regarding other social concerns possibly resulting from routinization, pregnant women were most concerned regarding a reduction in resources available for people with Down Syndrome and their families and least concerned regarding a decrease in the population of people with Down Syndrome.Conclusions Our findings reflect the concerns expressed by pregnant women and their partners, both personal (pressure to test, pressure to terminate) and societal (e.g., regarding potential negative impact on people with disabilities and their families). Even if most women were not concerned about feeling pressured to test due to NIPT routinization, a large minority express concerns that should not be taken lightly. Moreover, a majority of respondents were concerned regarding pressure to terminate pregnancies due to NIPT routinization as well as regarding most societal impacts they were queried on, especially the possible future reduction in resources available for people with DS and their families. Canadian policy-makers should consider these potential negative ramifications of NIPT and ensure that appropriate social policies accompany its implementation. (shrink)

A distinction has been drawn between basic conversation analysis and applied CA. Applied CA has become especially beneficial for informing areas of practice such as health, social care and education, and is an accepted form of research evidence in the scientific rhetoric. There are different ways of undertaking applied CA, with different foci and goals. In this article, we articulate one way of conducting applied CA, that is especially pertinent for practitioners working in different fields. We conceptualise this as Reflective (...) Interventionist CA. We argue that this approach to applied CA is important because of its emphasis on the reflective stance that is valuable to an understanding of research data, its commitment to collaboration with practitioners, and its inductive style. In this paper, we outline the core premises and benefits of this approach and offer empirical examples to support our argument. To conclude, we consider the implications for evidence-based practice. In everyday conversations, people put forward versions of events and provide supporting evidence to build a credible case. In environments where there are potentially competing versions, case-building may take a more systematic format. Specifically, we conducted a rhetorical analysis to consider how in child mental health settings, families work to present a credible ‘doctorable’ reason for attendance. Data consisted of video-recordings of 28 families undergoing mental health assessments. Our findings point to eight rhetorical devices utilised in this environment to build (...) a case. The devices functioned rhetorically to add credibility and authenticate the case being built, which was relevant as the only resource available to families claiming the presence of a mental health difficulty in the child was their spoken words. In other words, the ‘problem’ was something constructed through talk and therefore the kinds of resources used were seminal in decision-making. (shrink)

The lateralized ERP N2pc component has been shown to be an effective marker of attentional object selection when elicited in a visual search task, specifically reflecting the selection of a target item among distractors. Moreover, when targets are known in advance, the visual search process is guided by representations of target features held in working memory at the time of search, thus guiding attention to objects with target-matching features. Previous studies have shown that manipulating working memory availability via concurrent tasks (...) or within task manipulations influences visual search performance and the N2pc. Other studies have indicated that visual vs. spatial working memory manipulations have differential contributions to visual search. To investigate this the current study assesses participants' visual and spatial working memory ability independent of the visual search task to determine whether such individual differences in working memory affect task performance and the N2pc. Participants completed a visual search task to elicit the N2pc and separate visual working memory and spatial working memory assessments. Greater SPWM, but not VWM, ability is correlated with and predicts higher visual search accuracy and greater N2pc amplitudes. Neither VWM nor SPWM was related to N2pc latency. These results provide additional support to prior behavioral and neural visual search findings that spatial WM availability, whether as an ability of the participant's processing system or based on task demands, plays an important role in efficient visual search. Drawing on a landscape analysis of existing data-sharing initiatives, in-depth interviews with expert stakeholders, and public deliberations with community advisory panels across the U.S., we describe features of the evolving medical information commons. We identify participant-centricity and trustworthiness as the most important features of an MIC and discuss the implications for those seeking to create a sustainable, useful, and widely available collection of linked resources for research and other purposes.

Background This paper describes the design, implementation, and process outcomes from three public deliberations held in three tribal communities. Although increasingly used around the globe to address collective challenges, our study is among the first to adapt public deliberation for use with exclusively Indigenous populations. In question was how to design deliberations for tribal communities and whether this adapted model would achieve key deliberative goals and be well received.Methods We adapted democratic deliberation, an approach to stakeholder engagement, for use with (...) three tribal communities to respect tribal values and customs. Public deliberation convenes people from diverse backgrounds in reasoned reflection and dialogue in search of collective solutions. The deliberation planning process and design were informed by frameworks of enclave deliberation and community-based participatory research, which share key egalitarian values. The deliberations were collaboratively designed with tribal leadership and extensive partner input and involvement in the deliberations. Each deliberation posed different, locally relevant questions about genomic research, but used the same deliberation structure and measures to gauge the quality and experience of deliberation.Results A total of 52 individuals participated in the deliberations across all three sites. Deliberants were balanced in gender, spanned decades in age, and were diverse in educational attainment and exposure to health research. Overall, the deliberations were positively evaluated. Participant perceptions and external observer datasets depict three deliberations that offered intensive conversation experiences in which participants learned from one another, reported feeling respected and connected to one another, and endorsed this intensive form of engagement.Conclusion The adapted deliberations achieved key deliberative goals and were generally well received. Contemporaneously, tVNS can be achieved by stimulating either the auricular branch or the cervical bundle of the vagus nerve, referred to as transcutaneous auricular vagus nerve stimulation and transcutaneous cervical VNS, respectively. In order to advance the field in a systematic manner, studies using these technologies need to adequately report sufficient methodological detail to enable comparison of results between (...) studies, replication of studies, as well as enhancing study participant safety. We systematically reviewed the existing tVNS literature to evaluate current reporting practices. Based on this review, and consensus among participating authors, we propose a set of minimal reporting items to guide future tVNS studies. The suggested items address specific technical aspects of the device and stimulation parameters. We also cover general recommendations including inclusion and exclusion criteria for participants, outcome parameters and the detailed reporting of side effects. Furthermore, we review strategies used to identify the optimal stimulation parameters for a given research setting and summarize ongoing developments in animal research with potential implications for the application of tVNS in humans. Finally, we discuss the potential of tVNS in future research as well as the associated challenges across several disciplines in research and clinical practice. (shrink)

The sclera, the eye’s tough outer layer, is, among primates, white only in humans, providing the ground necessary for the display of colors that vary in health and disease. The current study evaluates scleral color as a cue of socially significant information about health, attractiveness, and age by contrasting the perception of eyes with normal whites with copies of those eyes whose whites were reddened, yellowed, or further whitened by digital editing. Individuals with red and yellow sclera were rated to (...) be less healthy, less attractive, and older than individuals with untinted control sclera. Individuals with whitened, “super-white” sclera were rated as younger, although not more healthy or attractive, than controls. In humans, clear, white sclera may join such traits as smooth skin and long, lustrous hair as signs of health, beauty, and reproductive fitness. The evolution of a white sclera may have contributed to the emergence of humans as a social species. (shrink)

In a globalized world, the causes, connections, and responsibilities become complicated. Agriculture subsidies that keep food prices low for many in relatively rich countries may, for example, negatively impact poor farmers in developing countries. Students in Ethics/Philosophy 352 report on their project examining whether, and to what extent, a true obligation to aid exists.

Com os processos de globalização e das novas tecnologias de informação, começam a surgir uma série de atores políticos que passam a exercer grande força política, econômica e jurídica, afetando diretamente os direitos humanos. A partir disso, pretende-se analisar a possibilidade de aplicação da técnica da autorregulação regulada como forma de garantir a proteção dos direitos humanos no ambiente das empresas transnacionais. Para isso, utilizando-se o método dedutivo, se abordará os elementos básicos do processo de globalização, o surgimento de novos (...) agentes supraestatais e a violação dos direitos humanos na lógica de mercado, para, por fim tratar da autorregulação regulada. O presente artigo tem o intuito de apresentar a concepção kantiana acerca do belo empírico e sua contribuição para o modo de civilizar humano. O primeiro ponto do artigo trata de contextualizar o interesse pelo belo empírico apontando a concepção de juízo estético e do belo em Kant a fim de contextualizar a abordagem kantiana sobre os elementos estéticos da obra Crítica da Faculdade de Julgar (1790). O segundo ponto introduz as características próprias concernentes ao processo de sociabilidade humana, trazendo (...) noções como a de sensus communis e comunicabilidade. O terceiro ponto trata de apresentar uma interconexão do processo de sociabilidade em Kant, exposto pela Terceira crítica, com outros textos kantianos que versam sobre história e antropologia. Com base nessa relação comparativa, o artigo mostra em que medida ocorre o melhoramento da sociabilidade humana a partir do interesse pelo belo empírico e como esse interesse contribui para a formação da uma sociedade civilizada. É justamente ao me narrar que me crio e me constituo enquanto (...) sujeito. Abstract Context: Established in 1997, Summa Health System’s Medical Ethics Committee (EC) serves as an educational, supportive, and consultative resource to patients/families and providers, and serves to analyze, clarify, and ameliorate dilemmas in clinical care. In 2009 the EC conducted its 100th consult. In 2002 a Palliative Care Consult Service (PCCS) was established to provide supportive services for patients/families facing advanced illness; enhance clinical decision-making during crisis; and improve pain/symptom management. How these services affect one another has thus far been (...) unclear. Objectives: This study describes EC consults: types, reasons, recommendations and utilization, and investigates the impact the PCCS may have on EC consult requests or recommendations. Methods: Retrospective reviews of 100 EC records explored trends and changes in types of consults, reasons for consults, and EC recommendations and utilization. Results: There were 50 EC consults each in the 6 years pre- and post-PCCS. Differences found include: (1) a decrease in number of reasons for consult requests (133–62); (2) changes in top two reasons for EC consult requests from ‘Family opposed to withdrawing life-sustaining treatment (LST)’ and ‘Patient capacity in question’ to ‘Futility’ and ‘Physician opposed to providing LST’; (3) changes in top two recommendations given by the EC from ‘Emotional Support for Patient/Family’ and ‘Initiate DNR Order’ to ‘Comfort Care’ and ‘Withdraw Treatment.’ Overall, 88% of recommendations were followed. Conclusion: PCCS availability and growth throughout the hospital may have influenced EC consult requests. EC consults regarding family opposition to withdrawing LST and EC recommendations for patient/family support declined. Este trabalho tem por objetivo analisar a figura de Rosa Luxemburgo como exemplo de ascensão das mulheres ao espaço público tal qual descrito por Hannah Arendt, e refletir sobre os motivos que permitiram seu negligenciamento histórico. Tendo em vista que apesar de ter tido grandes contribuições teóricas e práticas e ter ascendido ao espaço público, ainda assim Rosa Luxemburgo fora negligenciada, estereotipada por meio da figura ora sanguinária, ora romantizada, e por esses motivos Arendt acredita que a política alemã não (...) obteve o reconhecimento devido que mereceria nem em vida e nem em morte. Para tanto, utilizaremos a distinção entre espaço público e espaço privado de Arendt, presente na obra _A condição humana,_ bem como seu ensaio intitulado “Rosa Luxemburgo: 1871-1919”, presente na obra _Homens em tempos sombrios_ _ _e a obra _Rosa Luxemburgo ou o Preço da Liberdade_ de Jörn Schütrumpf_. _Por fim, corroboramos com Hannah Arendt e trazemos também Jörn Schütrumpf para reafirmar que apesar de ter ocorrido uma pequena mudança em sua reputação, e algumas pessoas até saberem o nome de Rosa Luxemburgo, diante de sua grande contribuição teórica e prática seu reconhecimento acadêmico e político ainda é insuficiente, e acreditamos ainda ser possível um reconhecimento ainda que tardio da importância da figura de Rosa Luxemburgo para o pensamento teórico/filosófico político contemporâneo. Este artigo é sobre a folksonomia como uma ferramenta que contribui para as atividades colaborativas de grupos de usuários na rede social acadêmica Passei Direto, com o objetivo de analisar a folksonomia como ferramenta de apoio em grupos colaborativos nessa rede social, por meio de estudo de caso holístico e exploratório, sendo realizada observação direta intensiva. A justificativa baseou-se na necessidade de compreender a potencialidade das atividades cooperativas e a solução das necessidades informacionais. Concluiu-se que a folksonomia é uma ferramenta (...) de apoio em grupos colaborativos no Passei Direto, onde os usuários, no uso de tags em práticas de indexação, visam a enriquecer e ampliar as possibilidades de acesso às informações. No entanto, apesar de representar uma ferramenta de forte cunho colaborativo, a folksonomia também deixa lacunas no tocante à recuperação da informação, dada a falta de padronização e controle dessa ferramenta, embora essas características façam parte da sua essência. O presente artigo parte da problemática: qual a forma de repressão estatal brasileira relativa às fake news? Como provável hipótese de pesquisa tem-se que o Estado brasileiro não tem sido eficiente em dirimir este problema, carecendo de regulamentação específica. O objetivo geral consiste em analisar a atuação do Estado em relação à contenção de fake news. Para tanto, utilizou-se o método de procedimento dedutivo e o de pesquisa bibliográfico. Como conclusão, percebe-se que o Estado brasileiro não consegue alcançar o fenômeno (...) das fake news e a repressão acaba ficando a cargo da iniciativa privada. (shrink)

The present article proposes an analysis of the foucauldians studies concerning the disciplinary techniques and its relation with the government of individuals and collectives conducts. To this end, this article focus in demonstrated, throughout the works “Discipline and Punish: The birth of the prison ”, “The order of discourse ”and “Micro-physics of Power ”, how the discipline, understood as a technology of power, operates in the conduct of the subject practices.Thus, we will follow an analytical path of differentiation of the (...) both uses of the term discipline found in the theorizations of Michel Foucault: one in the way of the knowledge, thenceforth the control of the discourses formation which configures the acting of a power that exercises itself as a discursive restrictor in order to maintain the regulation and control of what can be said and though in certain time. The other use, constituted in the scope of the power, in which the disciplinary power acts specifically in the bodies as a group of techniques engendered in the subjects’ fabrication. Therefore, the main objective outlines in present the discipline as a technology of the power the works as an machinery of the government of conducts, highlighting its forms of operation in the fields undertaken by Foucault, of the know and the power, following his genealogic triad of investigation. The article still intends to explore the foucauldian understanding that the discipline and the disciplinary techniques are configured as an ingenious machine that come together in the fabrication of the human, being required to understand them not only through the repression categories, oppression and alienation, like much has been done and continues to be made. O presente trabalho tem como objetivo discutir implicações da dialética do esclarecimento, discutida na obra homônima de Adorno e Horkheimer, para a educação. Para tanto, volta-se ao questionamento central da obra acerca da permanência da barbárie mesmo nas sociedades com pretensões democráticas e apesar do desenrolar do processo civilizatório e do notório progresso técnico-científico que caracteriza o nosso tempo. Os argumentos fundamentais da obra citada são retomados, relacionando-a ao contexto de sua produção. Elementos como as contradições que são imanentes à (...) cultura, a necessária autorreflexão do pensamento, a lida com o medo e com outros afetos decorrentes do não cumprimento da promessa emancipatória da cultura, a matematização da ciência e a expurgação do incomensurável e do subjetivo são pensados como implicações significativas da dialética do esclarecimento para o campo educativo. Discute-se que a educação, ainda que precariamente, é, por excelência, o âmbito tanto de formação das novas gerações como de produção de saberes, o que a torna rica em possibilidades tanto de fomentar os aspectos emancipatórios da cultura como de reproduzir e aprofundar as mazelas de nosso tempo, o que indica a importância desta reflexão. Procura-se, em alguma medida e com o intuito de aprofundar o escopo argumentativo, referenciar tanto outras obras dos autores frankfurtianos da primeira geração como obras de pesquisadores que se dedicam à atualização da produção teórica nesse campo de saber. À guisa de conclusão, são apontadas tarefas da educação, bem como aporias que precisam seguir sendo pensadas quando se concorda com a finalidade intrinsecamente emancipatória da formação.
